Start forms and hit F1:
From Forms help

1. In the Object Navigator, double-click the icon next to the desired button to display the Property Palette.

2. Under the Functional node:
- Set the Iconic property to Yes
- set the Icon Filename property to the name of the icon. When defining the Icon Filename property, do not include the icon file extension. For example, enter MY_ICON, not MY_ICON.ICO.

Note: At runtime, Form Builder searches for the icon in the path specified by the environment variable for your system (UI60_ICON on Microsoft Windows).
